Merton Uplift will work with people who have a long standing but stable mental health conditions who may require some short-term support to improve their quality of daily living, or reduce social isolation. We will also offer additional support with changing or reviewing psychiatric medication and support GPs with physical health needs.
A dedicated team consisting of nurses, recovery support workers, senior psychologists /psychotherapists, and led by a consultant psychiatrist will work together to ensure that people with long-standing and complex mental health conditions receive greater support with an emphasis on reducing symptoms and suffering and supporting clients to make changes in order to improve their ability to function and live more meaningful and fulfilling lives without having to be referred to a community mental health team.
