Join us at the Champions of Self-Management Conference
Merton CCG will be hosting a Self-Management Conference to promote self-management for patients, public, carers and health and social care professionals across Merton.
Wednesday 5th October, 12.30pm – 16.30pm
Merton Hall, 78 Kingston Road, South Wimbledon, SW19 1LA
Self-management is a term used to include all the actions taken by people to recognise, treat and manage their own health.
The Champions of Self-Management Conference will support the spread of learning and practice in self-management by:
Improving public knowledge of what self-management is for people living with long term conditions.
encouraging people living with long term conditions, and those who support them, to adopt a self-management approach.
working together with health and social care professionals to help and support people to make better decisions
building better community connections across Merton.
The day will include presentations and Health Hub Market stalls from different organisations that are all committed to making a difference to people living with long-term conditions.
The event is open to all and we are inviting people in Merton with an interest in self-management whether they work in health and social care, have a disability, long term condition or are an unpaid carer.
Lunch and light refreshments will be provided.
