The lead clinicians in the East Merton locality meet regularly to discuss ways of improving the health of our local population and to ensure that local services meet the needs of our patients.
We are committed to delivering high-quality care both within our individual practices and also by working collectively to provide some services traditionally only available in hospitals at some of our GP surgeries. An exciting development for the future is the provision of a local care centre in Mitcham, which will considerably enhance facilities for clinicians and patients and allow more patients to be treated in the community.
We are working closely with community services and social services with a particular focus on improving care for patients with long term conditions. Several of our practices have patient participation groups and we value patient feedback in shaping both current and future services.
The CCG is also establishing links with our local hospitals via the clinical quality review groups which take place monthly to monitor the quality of the services provided by our main local hospitals.
