​​​​​​​Join us at the Champions of Self-Management Conference​

Merton CCG will be hosting a Self-Management Conference to promote self-management for patients, public, carers and health and social care professionals across Merton.

Wednesday 5th October, 12.30pm – 16.30pm

Self-management is a term used to include all the actions taken by people to recognise, treat and manage their own health.

The Champions of Self-Management Conference will support the spread of learning and practice in self-management by:

  • Improving public knowledge of what self-management is for people living with long term conditions.

  • encouraging people living with long term​ conditions, and those who support them, to adopt a self-management approach.

  • working together with health and social care professionals to help and support people to make better decisions

  • building better community connections across Merton.

The day will include presentations and Health Hub Market stalls from different organisations that are all committed to making a difference to people living with long-term conditions.

The event is open to all and we are inviting people in Merton with an interest in self-management whether they work in health and social care, have a disability, long term condition or are an unpaid carer.

Lunch and light refreshments will be provided.
