Social Prescribing Day event on Thursday 14th March - Sold Out

Social prescribing is where GP practices signpost patients to non-medical treatments in the community such as walking groups or benefits advice.
Doctors and patients will be sharing their stories about the programme on the day organised by NHS Merton and Wandsworth Clinical Commissioning Groups.(CCGs)
You will have a chance to hear about a pilot project in Merton which improved the health and wellbeing of patients which resulted in a reduction of 33% GP appointments and 50% hospital visits.
At the event there will be a chance to network with colleagues, to hear about the successes and challenges of developing Social Prescribing in Merton and the ambitions for rolling out the model into Wandsworth.
The Merton pilot ran for a year in Wide Way Medical Centre and Tamworth Medical Centre in East Merton and supported people with anxiety and stress, and those experiencing social isolation and loneliness. A social prescribing co-ordinator based in the practice guides each patient to the best support for them in the community.
